1. Forward clutch plates burned.
2. Porosity-forward clutch piston.
3. Forward clutch seals, cut or damaged.
4. Damaged forward clutch housing.
5. Forward clutch internal leak.
6. Forward clutch housing check ball damage.
7. Low oil or oil pressure.
8. Accumulator valve sticking.
9. Valve body lands or interconnected passages damaged.
10. Valve body gasket, spacer plate damaged or improperly positioned.
11. Binding internal throttle valve (TV) linkage.
12. 1-2 accumulator piston assembly, piston or bore porous.
13. 1-2 accumulator piston assembly seals, cut or damaged.
14. Leak between piston and pin.
15. Missing or broken accumulator spring.
