I. Complete the Vehicle Campaign Notice

The Vehicle Identification Number Selling Retailer Code Campaign Number and Owner Name and Address are pre-printed.

Servicing retailers must complete the following items:

1. Servicing Retailer Code.

2. Repair Code.

3. Repair Date & Repair Order Number.

If you utilize a blank campaign notice all information normally pre-printed must be supplied by the servicing retailer.
