Modification Label and Proof of Correction Form Instructions

1. An authorized Modifications Label will need to be applied to the underside of the hood. Wipe the surface dean before applying the new label.

2. On the Authorized Modifications Label, you will need to write in the following information:
CHANGE AUTHORITY: 018
DEALER CODE: YOUR DEALER CODE
REPAIR DATE: THE DATE OF THE REPAIR

Remove the backing from the clear adhesive on the front of the label, and press down on the label.





3. Fill out the California Vehicle Emission Recall - Proof of Correction form as shown.
