1. Complete the AIPDN Return Request Form (AIPDN-119).

^ Enter Part No. 2-90496-800-0.

^ Include cost of shipping accessory rubber floor mat set back to facing PDC.

^ Check box in return request section of form for Special/Damage return.

^ Write in "Campaign Return - 1998 Rodeo Rubber Mats".

2. Ship the accessory rubber floor mat set (4 pieces total) and the completed AIPDN Return Request Form, along with the shipping invoice, to the facing PDC.
